Comments: The rice inbulk food containers has been chewed by rodents and the sugar in the basement is infested with roaches. Discard unsafe food products. (The employees placed the sugar and rice in trash bags, denatured the items with bleach and discarded the food in the dumpster out back.)
Comments: Chicken stored at room temperature 65F and food that is for employees is stored at room temperature. Store all food under refrigeration of 41F or below if the food is for employees store it in a separate clearly marker refrigerator or employee area.
Comments: The handsink in the back is blocked by equipment. Provide access to handwashing at all times. Employees are observed rinsing off their hands in the three compartment sink, they are not washing them correctly with soap and water or washing them in the correct sink. Properly train employees in handwashing and safe handling procedures.

Comments: Employees are observed eating in the kitchen. Train employees about proper hygienic practices.

Comments: The bulk food containers in the basement are soiled and chewed. Clean or replace containers.

Comments: There is cardboard lining the surfaces in the kitchen. Remove and provide a smooth, cleanable, durable surface.

Comments: The rice container has been chewed and two bags of rice inside the plastic barrel are chewed. There are cockroaches in the bulk sugar container in the basement. There is a gross infestation of cockroaches and rodents at the establishment. Provide increased treatment until the problem is resolved.

Comments: There are holes in the walls. Repair holes to help keep rats out of the establishmnet.
Comments: The certified food manager is not at the location but there is a long history of repeated risk factor violations and infestation at the establishmnet. Provide additional certified food manager to properly supervise the food safety at the establishment.
